Free CME on Cutting Hospital Readmissions on October 23

The Maryland Health Care Commission (MHCC) and MedChi invite you to join their next MHCC Learning Network event on October 23, 2026, where we will discuss innovative ways outpatient care reduces hospital readmissions by catching health issues early, addressing medication mistakes, and supporting patients after they go home.

The discussion will focus on how different team members support patients recently released from the hospital through timely follow-up visits, medication reconciliation, remote patient monitoring, telehealth, ongoing patient education, and addressing health related social needs that may disrupt healing.
